Immunization Requirements
Dear Parents/Guardians:
The Hyde Park Central School District recognizes that we are experiencing challenging and uncertain times in the midst of COVID-19. Guidelines remain dynamic and we appreciate your patience and understanding.
In an effort to continue keeping our students safe, the immunization requirements for school attendance remain the same.
All Students entering grades K, 1, 2, 3, 4, and 5th grades require the following vaccinations:
4 or 5 doses of DTap; If the 4th DTap dose was received at 4 years of age or older (otherwise a 5th dose is required)
3 or 4 doses of Polio; If the 3rd Polio dose was received at 4 years of age or older (otherwise a 4th dose is required)
2 doses of MMR
2 doses of VZV (chickenpox)
3 doses of Hepatitis B
We urge you to either contact your family physician to schedule an appointment, or the Dutchess County Department of Behavioral and Community Health Immunization Clinic, 29 N. Hamilton Street, Suite 109, in Poughkeepsie at 486-3409 for information regarding immunization clinics.
