All you need to do is go back into the Family ID program and into your family account - click on REGISTER NOW - pick the sport your son/daughter wants to play - click on your athlete's name, all the previous info will fill in - UPDATE the health form - resign all signatures - SUBMIT
New York State Education Department requires tht the health information be accurate within thirty days of the start of the season. When your updates have been made and your registration submitted, the nurses in the Health Office will complete the registration process.
Please check your email after submitting your registration for any information that might be missing. Missing items could include providing a physical within one year of the start of the season, providing a physician's order for medication needed during the season, or an electronic signature that is missing or incorrect.
