Central Student Registration-Main Page

PLEASE REVIEW ALL REQUIRED DOCUMENTATION BEFORE STARTING REGISTRATION!

To register your student, please follow steps 1-3.
 

1. FILL OUT THE REQUIRED FORMS

Please print out the appropriate registration packet from the list on the right.  You can always stop by the Central Registration Office located at 11 Boice Road for a paper copy, or assistance.

Additional Required Forms (if applicable):

• Application for Free & Reduced Price School Meals (only 1 application per household)*
• Request for Daycare Transportation
• Owner's Affidavit (must be notarized)
• Renter's Affidavit (must be notarized)
• Custodial Affidavit (must be notarized)
• Migrant Education Program Survey

*Your child may be eligible for benefits such as Medicaid or Children's Health insurance Program (CHIP). To determine if your child is eligible, program officials need information from your Free and Reduced Price Meal application.

2. SUBMIT FORMS 

Once you have completed the forms and gathered all of the supporting required documents, you will need to make an appointment to drop everything off for review.  Please call Central Registration at (845)229-4000 extension 1606 or 1607 or you may schedule an appointment online.

3.  SUBMIT REQUIRED DOCUMENTATION

Children must be 5 years old by Dec. 1, 2025 to be eligible for kindergarten enrollment in September 2025.

Students currently attending UPK at Hyde Park Elementary or Holy Trinity Annex had packets sent home with them in February.  Please be sure to complete a new registration packet for your UPK student!

Change of Address

If you are a current district resident and have moved within the district please contact the Registration Office at (845)229-4000 ext. 1606 or 1607 to schedule an appointment to change your address. You must bring the following documentation to your appointment:
● Government issued photo ID. for Identification Purposes (e.g., Driver’s License, Passport)
● Proof of Ownership or Rent as defined here.
● Two (2) Proofs of Residency as defined here.

Please review the following instructions for successfully completing a fillable PDF form.

✔ Use the latest version of Adobe Reader.

To open and complete the PDF application forms, you will need Adobe Reader. If you do not have it installed on your computer, you may download the latest version free of charge from http://get.adobe.com/reader/otherversions.

✔ Save the form to your computer before completing it.

Do not complete the form online within your web browser; your data will NOT be saved. Please save the form to your computer first and then fill it out. 
 
To save:

1. Position your cursor on the form link and click with your right mouse button (do not activate the link and open the form).

2. From the menu that pops up, select the Save As option.

3. You should then be prompted to choose a location to save the file.

4. Select the location on your own computer or network and click on the Save button.

5. Once saved, navigate to the file and type directly into the form fields and save your work as you go. You can save your data and re-open the file later to modify or enter additional information. 

✔ Print only when your cursor is not in a form field.

Use your mouse to select an area of the form that is not inside a form field or hit the tab key after completing the entry of your last box before printing your form. If a form field is active (e.g., contains the blinking bar) the contents of that block will not print.

✔ Print from Acrobat, not the browser.

If the fillable form is displayed within your web browser’s window be sure to use the printer button on the Acrobat toolbar menu to print the form instead of your web browser’s print function.
